CHAP is an independent non-profit accrediting body for community-based health care organizations. Created in 1965, CHAP was the first to recognize the need and value for accreditation in community-based care. CHAP is the oldest national, community-based accrediting body with more than 9,000 agencies currently accredited nationwide. Through “deeming authority” granted by the Centers for Medicare and Medicaid Services (CMS), CHAP has the regulatory authority to survey agencies providing home health, hospice, and home medical equipment services to determine if they meet the Medicare Conditions of Participation and CMS Quality Standards. CHAP’s purpose is to define and advance the highest standards of community-based care.

Executive Team

Usman Jilani, M.D., M.H.A. – President

Dr. Jilani is an accomplished physician and healthcare executive with over twenty years of experience. Dr. Jilani applies lessons learned from leadership roles in medical/surgical, project management, consulting, and international technology to optimize clinical services and systems in support of excellence in patient care.

In addition to his experience, Dr. Jilani holds an MD from St. Matthew’s University School of Medicine, a Master of Health Services Administration from Saint Joseph’s College, and earned his undergraduate degree in business from Loyola Chicago.

After completing a fellowship in nephrology from Henry Ford Hospital, Dr. Jilani worked in primary care for many years and led the clinical informatics team at Tenet Healthcare in Chicago. Dr. Jilani served numerous roles within the Tenet Healthcare system in Chicago to ensure Healthcare Informatics success by securing physician ownership, C-suite sponsorship, training support, and organization-wide adoption of change. Dr. Jilani has been pivotal in some of the leading health systems across the nation: The Iowa Clinic, Texas Healthcare, USMD, Advocate Aurora Health, Rush University Medical Group, and Lehigh Valley Physicians’ Group.

Dr. Jilani’s career has enabled him to develop a mix of skills, spanning from clinical and strategy to systems, and offered healthcare systems with advisory and support on critical initiatives.

Maria Jilani, R.N. – Senior Vice President

Mrs. Maria Jilani is a registered nurse specializing in nephrology and dialysis, with over 15 years of experience in the medical and healthcare field.

Maham Jaffry – Business and Strategic Consultant

Ms. Maham Jaffry is a young and talented leader that always puts her clients first. Ms. Jaffry received her bachelor’s degree in business management with a focus on adaptable business management and leadership from DePaul University. Ms. Jaffry has many years of market experience, aiming to leverage a proven knowledge of customer retention, customer communications, leadership, problem-solving, prioritizing, coachability, and customer satisfaction skills.
